Out-patient admissions

 

Out-patient admissions may involve medicinal, surgical or oncology treatment.

Pre-admission formalities

When the hospitalisation has been planned in advance:

  • For surgery: pre-admission is completed after the consultation with the anaesthesiologist
  • For medical treatment, pre-admission is completed after the consultation with the specialist

The patient will complete their pre-admission at the ground floor where the admissions office is open Monday to Friday:

  • 7:30am to 5:30pm at the Kléber site
  • 8:30am to 5:30pm at the Barbès site

Please remember to bring the following documents:

  • Proof of identity with photo (ID card, passport, residency permit)
  • Your ‘Vitale’ card or proof of valid healthcare coverage
  • Your CMU/CMUC/AME certificate
  • Your proof of 100% coverage
  • Your valid top-up insurance card,
  • Your blood-group card (if you have one),
  • Your proof of application for coverage from your top-up insurance provider or private healthcare insurance provider (with details of the coverage).

Special cases:

  • For foreign nationals: your European health insurance card or E112 form or a private healthcare insurance certificate stating the validity period and the types of treatment covered.
  • If you do not have French social security coverage, you should ask for an estimation of your hospital fees and pay them in advance.
  • Senior citizens: our staff will help you to complete the formalities.

Admission and departure formalities

  • On the day that the patient is admitted to the hospital, they should go to the admissions office to validate their PRE ADMISSION and pick up the labels to be given to the unit or ward staff in question; unless you are admitted as an oncology out-patient, in which case you will be asked to go straight to the ward or unit.
  • On leaving the hospital, the patient will go to the admission office to complete the departure formalities (invoicing and payment); where they will receive the hospitalisation summary.

Outside of admissions office opening hours these formalities will be completed on the following day and all of the documents will be sent to the home address.
